My employer doesn't seem to realize that my caring for my disabled adult sister affects my work, just as the work of my co-workers with young children is affected by them. How can I raise the issue without drawing negative attention?

Be open and honest with your employer about the roles and responsibilities you have as a caregiver. You may even compare it to having a young child who gets sick in child care, a school-age child who has an accident on the playground, or a teen who has an orthodontist appointment.
