Bearded iris need to be planted shallowly, with half the rhizome in the ground, and half exposed. Mulch should not be used around them. If planted deep or mulched, the rhizomes will rot. Remove the mulch, make sure they are planted shallow enough and discard any bulbs showing signs of decay. Another possibility is that the rot may have been caused by bacteria entering the wounds left by iris borers.
