Ask Your Doctor for an Immunization Record Card to Keep Track of Your Child’s Shots

Parenting Tips for Your 13-14 Month Old Toddler

When your child registers for school or childcare, most programs will ask for a copy of your child’s vaccine or immunization records. It’s important to keep an updated list of the vaccines your child has received and the dates he received them.

Your pediatrician or doctor may give you an immunization card when your child has her first vaccines. Put the card in a safe place, and be sure to take it to each doctor visit. If your doctor does not provide a card, you can print out a Vaccine Administration Record.

Use it to keep track of your child’s vaccines. Ask your pediatrician if she participates in an immunization information system (IIS), or immunization registry. A registry is a centralized database that stores all of your child’s vaccine records, even if she goes to more than one doctor. This way, even if you move or lose your record, there will still be a record in the immunization information system.
