Root weevils (rhododendron weevil, black vine weevil). This is a very common insect problem that causes disfiguration to plant tops when adults feed. Grub feeding on root crowns can weaken and kill plants. Best control is achieved by spraying adults. Contact your local Extension office for recommendations on insecticides and timing for applications. Weevils like to hide in dark areas during the day, and feed at night. Keeping the amount of leaf litter reduced underneath the plants will help.
