Should I as an adult wear a helmet to ride a horse?

Yes!

Head injuries comprise 18% of horse riding accidents. Traumatic brain injuries (TBI) are the number one cause for hospital admissions following a riding accident. Sixty percent of the deaths in horseback riding accidents are the result of TBI.

You should purchase a helmet designed specifically for horseback riding and that is approved by the American Society for Testing and Materials (ASTM). It is also important that the helmet be fitted properly to prevent injury.
