I would like to request my free annual credit report via U.S. mail and not by telephone or by typing personal information into a Web site. Is there a special form that I need to use to request my credit report?

Consumers can request a free copy of their credit report annually from each of the “big three” credit reporting agencies (Equifax, Experian, and TransUnion) through a centralized source developed to provide information and fill orders for free annual credit reports. Although the centralized source is a special Web site (www.annualcreditreport.com), a consumer has three options for receiving the free credit report: 1) through the Web site itself 2) via telephone by calling toll-free (877) 322-8228, and 3) by mailing a completed form to the following address: Annual Credit Report Request Service, P.O. Box 105281, Atlanta, Georgia 30348-5281. The form is available for downloading at the centralized source Web page: https://www.annualcreditreport.com/cra/order?mail.
