How is the adoption process different when the state is involved in the life of my grandchild?

The adoption process is basically the same whether the state is involved in the case or not. The main difference is that if a state case results in the termination of the parental rights, the grandparent does not need to get consent to adopt from the parents. Rather, the consent must come from the state licensing child-placement agency to whom guardianship and custody were transferred after the termination of parental rights.
