An Application for Replacement of Naturalization/Citizenship Document (Form N-565) is used to request citizenship and naturalization papers. You can download it from http://www.uscis.gov/files/form/N-565.pdf, write for a copy, or visit your nearest field office of the Bureau of Citizenship and Immigration Services. To find out which office is closest to you, access the USCIS Service and Office Locator on the Internet or call the customer service center at 800-375-5283. The sizable fee required to process the form can be paid only by check or money order.
